About NPPEHA

National Program for Prevention and Eradication of Heart Attack (NPPEHA) is a not for profit study and prevention program on the cardiac health of Indian citizens and the national burden in terms of Non Communicable Diseases. Our aim is to eradicate heart attack from India and our conviction is that “if polio can be eradicated, then why not Heart Attack?

At present, in every 10 seconds, 1 person is dying out of heart attack in India. India now has 60 million heart patients and by 2025 it will be 180 million as per a WHO study. Every day 900 youths under the age of 40 years are dying out of heart attack. Heart attack is the biggest widow maker of our nation.

India is the second most populous country in the world with over 1.3 billion people (May 2016), more than a sixth of the world’s population. Already containing 17.5% of the world’s population, India is projected to be the world’s most populous country by 2022, surpassing China. India’s population is likely to reach1.6 billion by 2050. India’s population growth rate is 1.2%, ranking 94th in the world in 2013.

India has more than 50% of its population below the age of 25 and more than 65% below the age of 35. It is expected that, in 2020, the average age of an Indian will be 29 years. In our opinion, India can no more boast that it is the only country in the world that has the highest number of young population, unless we have them in good health and with good lifestyle habits. In every occasion, world’s leading economic thinkers are praising and drawing growth theories in economic development, stating its huge young population. But they never considered the health status of India’s young generation, which is going to boomerang the country’s development dreams to nightmares.

The current scenario is largely disappointing. We are fast becoming a country with more and more of unhealthy young citizens. Can you ask a youth to jump 30 times standing in one point and then speak to you without gasping for breath? The truth is, 1 out of 3 young people in India are having 25 year older heart than their age. If we don’t control the wide abuse of addiction to tobacco and liquor with our young generation, India will be lost forever. Our education system is also faulty to some extent as it doesn’t promote the idea of healthy life and fitness among the (as well as to elders). Let us work together for a change; at least, we might be able to protect about 30 million babies from this trap, who’ll be starting their schooling by 2020.

NPPEHA aims to screen 60 million nationals across India in 3 phases, commencing from September 2016 to 2025. The free screening involves free medical camps to check serum lipid and sugar profile, blood pressure, pulse and bio metric index of every attendee. The data will then be entered and stored into a cloud based medical data base named “Heart of India” for studies and further follow-ups. 1St phase study will be over by December 2016 with a data of over 10000 people across India, which will further help us fine tune the programme for mass penetration and effectiveness in the following phases. Through NPPEHA, we’re also trying to bring about a massive change in the attitudes of families in terms of diet, health watch and fitness, the 3 mantras for prevention of heart conditions.

The NPPEHA programme is designed with consideration of India’s more than two thousand ethnic groups in which every major religion is represented with its distinct characteristics in lifestyle ranging from food to day to day living habits.   They are divided into four major families of languages (Indo-European, Dravidian, Austroasiatic and Sino-Tibetan languages) as well as two language isolates (the Nihali language spoken in parts of Maharashtra and the Burushaski language spoken in parts of Jammu and Kashmir). So the complex communication and mass media tool made for the NPPEHA campaign has addressed all these vital issue for maximum effectiveness using world class behavioural psychology tools.

Further complexity is also addressed by the great variation that occurs across this population on social parameters such as income and education. The special education package on nutritional dietary is made with the calculation of all economic groups with a nationwide study on available nutritional food items and its prices. NPPEHA is a well-studied Prevention and eradication program for Heart Attack considering all these above factors.

Normal range of Triglyceride should be less than 100mg/dl for healthy heart. Oil is known for the high amount of Triglyceride. High amount of Triglyceride is the major reasons for Heart Attack and build-up of plaques in the artery.

LDL Cholesterol & ratio with HDL

LDL Cholesterol is also called bad cholesterol because it is the main cause of blockage of coronary arteries. It play major role in development of Atherosclerosis or blockages in the arteries.

HDL Cholesterol is called good cholesterol because it carries cholesterol from other body parts to our liver and it’s the duty of liver to remove unwanted cholesterol from the body.

Ratio of HDL vs Cholesterol should be > 25%.

Systolic BP

It is the pressure which generate at the time of heart contraction.    Normal systolic BP is lesser than 120mmhg.

Diastolic BP

It is the pressure which detect at the time of heart rest. Normal diastolic BP is less than 80 mmhg.

Fasting Blood Sugar

Blood glucose is the essential component for our body to give cell energy. If blood glucose level increase in our body and not utilized by body cells then this condition called Diabetes (Hyperglycaemia).

Blood sugar fasting normal range is 70-110mg/dl.

Postprandial (PP) Blood Sugar

Blood Postprandial means level of glucose after meal. It is determine to carbohydrate absorption, insulin and glucagon secretion and their effect on metabolism of glucose.

Blood sugar PP normal value is 80-120mg/dl.

Smoking/ Tobacco

Smoking and chewing Tobacco are the major risk factor for heart attack and other serious life threatening diseases such as Stroke, COPD, Cancers etc. So you have to give up smoking and tobacco completely.

Weight (BMI)

Body weight should be under control and regular BMI should be measured to manage obesity and related disorders such as Hypertension, Diabetes & Heart disease. BMI is calculated with a person’s height, weight and the measurement of Waist and Hip.

Walking

Daily walking is very essential to be fit, active and healthy. So irrespective of your other fitness workouts, everyone should walk daily for at least 35 minutes a day to regulise body metabolism and to keep them fit. .

Zero oil food

Food with high percentage of oil is very bad for heart and other body parts. So you have to avoid oil and should consume oil free diet for reversal of coronary artery disease and other metabolic syndrome.

Milk & Dairy

Milk and dairy product contain high amount of fat and is an animal excretion, so it is classified into Cholesterol.  Dairy is bad for heart. So it is advised to consume double toned milk and its product for good health.

Fruits & Vegetables :

Fruits and Vegetables contains high amount of fibres and great source of vitamins, minerals & antioxidants, which are essential requirement for our body. So you have to take everyday at least 1-2 seasonal fruits to keep you healthy.

Vegetarianism

5. Add the tamarind paste and mix well.

5. Turn the tawa and cook the roti from the other side.
6. Remove from tawa and serve hot. Apply butter on top for butter tandoori roti.

In cooker :

For blanching:
1 bunch of Spinach leaves
2 inch ginger piece crushed or grated
7-8 garlic cloves crushed or grated
2 green chillis each cut into 3-4 pieces
water

For Gravy:
1 medium sized onion chopped finely
1 medium sized tomato chopped finely
salt to taste
1tsp red chilly powder
200 gms cottage cheese chunked

Dry Roast:
2 bay leaves
3 inch long cinnamon stick
1tsp cumin seeds

Recipe :

1. Clean Spinach leaves and cut the roots. Wash in water 2-3 times to remove mud from the leaves.
2. Fill a deep pot half with water, add pinch of salt and sugar. Add spinach leaves, cut chillis, crushed ginger and garlic to it.
3. Take a boil then switch of the gas, cover and keep for 5 mins. Strain the water and run the blanched leaves under cold water. Cool completely and make puree.
4. Now in a pan or kadhai add dry roasted cumin seeds, bay leaves and cinnamon sticks.
5. Add chopped onion roast till it turns light brown, add chopped tomato and roast till the raw smell is gone and the gravy comes together.
6. Season with salt and chilly powder at this step to prevent the gravy from burning.
7. Add pureed spinach and mix well. Add water if required till you get the desired thick consistency.
8. Chop the cottage cheese into chunks. It can be added directly to the gravy like this else you can lightly toast them on griddle or tawa and add to the gravy.
9. Serve hot with fulka or parotha.

If this clot cuts off the blood flow completely, the part of the heart muscle supplied by that artery begins to die.

How can I recover?

Depending on the extent of your heart attack, you may only be in the hospital a few days. But your recovery is just beginning.

It’s very important to start taking steps now to get your heart in good shape and to avoid another heart attack. Eat a healthful diet, exercise, and if you smoke, quit.

Talk with your doctor and nurses about how you can live as normal a life as possible. Ask how soon you can go back to work, drive a car, have sex, and what to do if you have chest discomfort. Your doctor or nurse can answer your questions about other matters, too.


How can I avoid a heart attack?

Even if you have heart disease, there’s a lot you can do to improve the health of your heart. Making changes can prevent a heart attack. Ask your doctor or nurse for help and information about how to change.

Don’t smoke.

Treat high blood pressure if you have it.

Eat a diet that’s low in fat, cholesterol and salt.

Eat lots of fresh fruits and vegetables.

Exercise.

Keep your weight in the normal range.

Follow your doctor’s orders for taking medicine.

See your doctor for regular checkups.

Control your blood sugar if you have diabetes.
You can help prevent a heart attack! You must control your blood pressure, stop smoking, eat a healthful diet, exercise and know the warning signs!

What Is Coronary Bypass Surgery?
Coronary Artery Bypass Surgery is a major heart operation of by passing the blocked arteries in the heart by putting veins removed either from legs of patients or any other part of the body so that blood can flow through the new arteries to the heart the way it should.

Almost half a million Coronary Bypass operations are performed each year, which are expensive costing between 1.5-2 lacs. This surgical procedure is only a temporary solution as half of the patients come back with reblockages within 3-5 years.
How is it done?

Your doctor will take a blood vessel from your chest or from your leg.

One end is attached to your aorta (the large artery that comes out of the heart), and the other end is attached to the coronary artery below the point where it’s clogged.

Blood can now flow through the new channel to the heart.
What is surgery like?

You will be asleep during the operation. It can take 3 to 6 hours.

After surgery, you go to an intensive care unit (ICU) for several days.

Your family can visit you briefly in ICU.
What about after surgery?

You’ll wake up in ICU and may feel confused at first.

It’s busy in ICU and the lights are always on. It’s normal to lose track of time.

You’ll have a tube in your mouth and throat to help you breathe. It’s uncomfortable and you can’t talk with it, but nurses will help you communicate.

The breathing tube will stay in until you can breathe on your own – in about 24 hours.

Cause of attack?

Almost all the heart attacks are sudden in onset and the cause is a rupture of the growing blockages. The blockages are usually covered by a thin membrane called Intimal membrane, which also keep the deposits intact. This elastic membrane gets more and more stretched as more and more fatty deposits occur. But if the fatty deposits continue below this permeable membrane, one day the time will come, when the membrane cannot stretch further and breaks off. This leads to the formation of a clot or thrombus inside the tube, closing the lumen completely. This completes the process of heart attack. The area of the myocardium (heart muscles) which gets the blood through the closed artery, dies – in the event of no blood supply.

Heart attack, often occurs after a heavy meal full of fat, after sudden anger or excessive sorrow or grief or excessive stress. Heart attacks occur more frequently in the early mornings. Heart attacks also occur during the process of Angiography and Angioplasty when the catheter or balloon is inflated and completely blocks the lumen of the coronary artery or breaks off the blockage by mechanical means.

Bio-Chemical Angioplasty
The concept
When we visited a chemical factory sometime back I was told when the chemicals pass through a pipe they react with the wall of the pipes and create plaque like obstructions. Ultimately the pipes get choked from inside. When I asked about the remedy the engineer in charge told me they send some opposite chemicals in the same tube and the plaques are removed. It is something like a housewife puts drainex in the kitchen drain to clear out the choked pipe.

Similarly, it looks logical that something that deposited can also be removed. A mixture of biochemical (all safe for the body and in correct dosage) now has shown results of reduction of coronary blockages when passed through the body. These chemicals mainly contain chemicals like Anti-oxidants, EDTA, Vitamins, Isotonic carrier, PH balancing drugs. All of them are injected in the patient through intravenous route for a period of two and half hours to obtain softening of the blockage and a gradual reduction of blockage. The results become apparent within six to ten infusions.

The dosages of the chemicals vary from person to person; and an experienced doctor has to decide which patients could be treated with this treatment. Has this therapy been proved beneficial? Though this kind of therapy is being used by a lot of practitioners abroad and in India using different kinds of chemicals it has not been promoted by the heart hospital system. The main reason being that this is not so lucrative. Though it was not so effective initially the improvement came after the chemicals used to soften the blockage and withdraw it, was changed. Today what Saaol uses is the best of the combination.

High blood pressure

Another name for high blood pressure is hypertension. (High blood pressure means the pressure in your arteries is consistently above the normal range. No one really knows exactly what causes most cases of high blood pressure. It usually can’t be cured, but it can be controlled.

Investigating-Facts about the heart

Investigation the rate of deposition of cholesterol in the lining of the blood vessels or the breakdown of the deposits (reversal of blockages) can be predicted. The normal values of these tests are given in the table below, but I would always recommend people not to keep their value at the upper end of the range but to keep them at the lower end. For example, if the range of Cholesterol is between 130 to 200 mg/dl – it is good to have a value which is near to 130 mg/dl. This becomes more important for those who already have blockages in the heart arteries or those who have other risk factors which they cannot control, due to circumstances.
Normal Range

  • Cholesterol                         130 to 200 mg/dl
  • Triglycerides                       100 to 160 mg/dl
  • HDL (good) Cholesterol       30 to 60 mg/dl
  • LDL (bad) Cholesterol          50 to 120 mg/dl
  • VLDL Cholesterol                 12 to 35 mg/dl

 

Unfortunately enough, I have been seeing numerous heart patients being prescribed all sorts of blood tests in the cardiology hospitals other than this most important blood test. Even angiographies and bypass surgeries are being done on these patients without even looking at the lipid profile.

It is a routine procedure to collect so many blood samples in the hospitals from heart patients and do tests like Australia Antigen, AIDS Sero Tests, Urea, Creatinine, Total and Differential blood Counts, Urine Tests…. and so on. Most of these tests are done in order to prepare the patients for procedures called Bypass surgery or Angioplasty. I have seen lipid profile reports still missing.

Lipid Profile Report
Since the heart disease is caused by fats, which is existing in different forms in the blood stream – heart patients are recommended to get a Lipid Profile Test. This is a blood test where the parameters usually tested are – Cholesterol, Triglycerides, HDL Cholesterol (good Cholesterol), LDL Cholest erol and VLDL Cholesterol. Blood has to be collected after 12 hours of fasting in this test.

Angiography

In Angiography, a long wire called catheter is inserted inside the artery of the leg, near the thigh crease. This catheter is then pushed against the blood flow towards the heart blindly. With a view of the tip of the catheter on the fluoroscopy monitor (which exposes the patient to very heavy radiation) this catheter is pushed onwards by trial and error method. If it gets stuck somewhere on the route, it is withdrawn a little and again pushed in. Not only it scratches the whole length of the arterial tubes of the body but it can also puncture any corner of the tubes. Once the tip reaches the heart area, further manipulation is done to push the tip in one of the coronary arteries. Once inside the coronary tube, after a lot of trial and error, a radioactive dye is injected through the hole in the catheter inside the coronary tubes and further fluoroscopy photographs are taken.

The tip of the catheter is again withdrawn, negotiated inside another coronary tube and the same photos are taken. If the dye seems to fill up the coronary tubes completely, the blockages are probably not there. It the dye can not fill the tubes (as roughly seen in the photos taken) inside, it is taken as filling defect and indirectly interpreted as blockages. The viewer mostly puts a rough percentage.

This report, being an eye estimation is given as 70%, 80% and so on. It varies from one viewer to another. It also depends on the timing of the photograph (best is before the dye is washed out), angle of the photograph etc. and is thus amenable to lot of different reports. It is not at all accurate and thus given in variations of 10%. One of my patients came to me the other day and told me that in the last two days he has already reversed his heart blockage by 10%. I asked “By what method”?. He said- “the second cardiologist told me that the blockage was 70%. Till now it was 80% according to the first cardiologist!”

Accuracy of Angiography

All the time you must have noticed the blocks are reported on 70%, 80%, 90% and so on. Why should they jump by 10% each time! This only shows how rough estimates are generalised and made a round figure. Ten to twenty percent variations are also there depending on the individual bias or variability of the cardiologist concerned. Angiographies are casually performed in India in most hospitals and are highly inaccurate. One day one patient told me “Sir, only yesterday I did 10% reversal of blockage. The cardiologist who I consulted last, said after seeing the film the blockage is not 80% but 70%”

Confirmation of attack

During the heart attack the muscles of the heart die, as they are not getting any blood supply. As the Cholesterol Blockages cannot be removed (which form 70-80 %), the blood clot removal is the only source of relief for the patients – who are in the face of losing heart muscles.

Immediate injection of clot busters like Streptokinase, Urokinase (very expensive injections) or drip of Heparin is the only possible remedial measure. If they are able to break the clot ( which usually closes 20-30% of the lumen of the tube), the heart muscles would be able to get at least some blood immediately and will survive if administered within one hour of the attack these injections can nullify almost all the damages of the heart attack. It will still have some effect if given within next five hours. But many a time these injections fail to deliver results. These medicines also have their side-effects like internal bleedings, cerebral hemorrhage, peptic ulcer bleeding and can be dangerous sometimes.

The damaged area of the heart causes a strain on the other live areas of the heart muscles. And it is very essential to avoid further heart attacks. So, after a heart attack, the patients are advised to stay in the hospital for a period of about seven days, the first few days being in the ICCU (Intensive Coronary Care Unit). Here, it is possible to take care of any emergency.

Early mobilisation of the heart attack patient, starting from the fifth day, is the latest concept of treatment. Previously it was seen that heart patients were not allowed any movement for the next one month or so. Now the dictum has changed, as with early movement (walks, exercises) the recovery has been found to be better.

Heart Attack?

Since most of those deaths are from heart attack – it’s important to learn all you can about heart attack. Don’t ignore heart problems. It’s a matter of life and death! For example, you should know the warning signs so you can get help right away, either for yourself or someone close to you. Acting quickly can save many, many lives! Uncomfortable heavy feeling, pressure, pain or squeezing in the center of your chest that lasts more than a few minutes.

What causes heart attack?
Another name for a heart attack is myocardial infarction) – “M.I.” for short. It occurs when the blood flow to a part of the heart is blocked (often by a blood clot). This happens because coronary arteries that supply the heart with blood slowly become clogged from a buildup of cells, fat and cholesterol called plaque Blood that tries to flow through these clogged arteries can form a clot. If this clot cuts off the blood flow completely, the part of the heart muscle supplied by that artery begins to die.

How can I recover?

Depending on the extent of your heart attack, you may only be in the hospital a few days. But your recovery is just beginning.

It’s very important to start taking steps now to get your heart in good shape and to avoid another heart attack. Eat a healthful diet, exercise, and if you smoke, quit.

Talk with your doctor and nurses about how you can live as normal a life as possible. Ask how soon you can go back to work, drive a car, have sex, and what to do if you have chest discomfort. Your doctor or nurse can answer your questions about other matters, too.


How can I avoid a heart attack?

Even if you have heart disease, there’s a lot you can do to improve the health of your heart. Making changes can prevent a heart attack. Ask your doctor or nurse for help and information about how to change.

Don’t smoke.

Treat high blood pressure if you have it.

Eat a diet that’s low in fat, cholesterol and salt.

Eat lots of fresh fruits and vegetables.

 

Exercise.

Keep your weight in the normal range.

Follow your doctor’s orders for taking medicine.

See your doctor for regular checkups.

Control your blood sugar if you have diabetes.
You can help prevent a heart attack! You must control your blood pressure, stop smoking, eat a healthful diet, exercise and know the warning signs!
